Forms Items/Fields

Important

Information and features vary according to the roles to which you belong and the permissions associated with those roles. For more information, contact your module manager or your campus support team.

There are several types of form components items that can collect information on your forms, including buttons, textboxes, drop-downs, and checkboxes. Form items are organized into rows and can be arranged to match your preferred order of appearance.

Question/Field Type

Description

Button

Commonly used for cause and effect. When the form completer clicks the button, they cause something to occur. Common form buttons include navigation buttons that save a user's progress as they go, save buttons to save progress, or submit buttons to finalize and send the form to the appropriate location. To add this functionality to buttons, you will need to set up rules for each button on the Rules tab of the button properties.

Checkbox

Form completers select one or more answers from a list of choices.

Date / Time

Form completers choose a date and time using a pop-up calendar.

Drop-Down

Form completers select an answer from several choices. This item/field type is ideal for presenting a long list of options without cluttering your form.

Essay

Form completers can enter multiple lines of text. For example, form completers can respond to essay question or enter open-ended comments/feedback.

Hidden

Form completers add information to your form that only other form builders can see. You can include instructions for setting up the form.

Label

Standalone text and typically used for providing instructions to form completers. It does not collection information; it only displays information to the form completer.

Placeholder

Adds a blank space to the form. This item type does not collect information.

Radio Button

Form completers select one answer from several choices.

Textbox

Form completers enter a single line of text. For example, completers can enter responses to simple questions.

How To

  1. Access the Connect hub, Forms module, Manage Forms page.

  2. Find the form you're updating and from the Actions menu, select Edit form.

  3. Find the item/field you want to move, click and hold the drag and drop icon i_drag_drop_block.png, and move the item/field to the desired position.

  2. Find the form you're updating and from the Actions menu, select Edit form.

  3. Click on the tab you want to update and select Edit.

  4. << When you're finished, click the OK button >>

  2. Find the form you're updating and from the Actions menu, select Edit form.

  3. Click on the tab you want to move, hold, and drag to position it in the desired order.

  1. Access the Connect hub, Forms module, Manage Forms page.

  2. Find the form you're updating and from the Actions menu, select Edit form.

  3. Find the item/field and click the ellipsis.

  4. Select Remove. A confirmation pop-up window appears.

  5. Click Yes. The confirmation pop-up window closes and the item is permanently removed.
