The Create Note button is available if the Submission Type is one of the following:

When you select a submission row and click the Create Note button, the submitted information is inserted into the Item Description window as a Notepad note. You can then edit as needed and also select the Display on Web checkbox if desired. When you click the OK button, a new note using the specified text and the Display on Web value is added to the ITEMS table for this ID Number. Also, the Submission row is automatically moved to a History status so that duplicate entries cannot be added to the ITEMS table for the same submission.
