The header of the statement includes the following:
· Your organization information (This information can be in the form of a logo that is preprinted on the form that you order from the print shop. Other statements have the computer print the name, city, state, ZIP Code, and phone number of your organization as they are in the A/R Subsidiary Type Control table.)
· Student information (name, ID Number, and address)
· Student payment due date
You should check each sample form to see if it is designed for a preprinted logo or computer-printed information.
