Hazlewood Recipient Information

Use this window to track service member and exemption hours information specific to students who receive the Hazlewood Exemption, a Texas State program for veterans. You can track and maintain the student's Hazlewood Exemption hours by year and term. Students eligible for the Hazlewood Exemption may be service members or their spouses or dependents.

The Hazlewood Recipient Information window is available only to institutions licensed for Texas State Reporting. If you have the proper permissions, access the window through one of the following methods:

· Click the Maintain Hazlewood Recipient Details button from the Student Billing Exemptions window.

· Click the Maintain Hazlewood Recipient Details button from the Military Service sub-tab of the Biograph tab on either the HR Employee Master via Personnel or HR Employee Master via Payroll window.

Columns

Displays the ID number and name of the person displayed on the previous window (Student Information, HR Employee Master via Personnel, or HR Employee Master via Payroll.)

Select the student's relationship to the service member. If the relationship is "Self," the student/service member's biographical information displays automatically.

	If the relationship you need is not available, select Maintain Definitions from the right-click window to open the Maintain Definitions window where you can add or edit the drop-down list values.
	Keep in mind that the values in this drop-down list are to be reported to the State of Texas as part of the Hazlewood Exemption Database Report. If you change any values, you must map each to a valid value required for the report. You can use the Institutional Reporting Definitions window to define Institutional Reporting fields and map them to J1 values.

Enter the service member's first name. If the Relationship to Service Member is "Self," this column is automatically populated.

Enter the service member's middle initial. If the Relationship to Service Member is "Self," this column is automatically populated.

Enter the service member's last name. If the Relationship to Service Member is "Self," this column is automatically populated.

Enter the service member's Social Security number. If the Relationship to Service Member is "Self," this column is automatically populated.

Enter the service member's date of birth. If the Relationship to Service Member is "Self," this column is automatically populated.

If the service member is a Texas resident, select this checkbox.

Enter the service member's zip code. If the Relationship to Service Member is "Self," this column is automatically populated.

If the service member is in default on any student loans, select this checkbox.

If the service member entered the service in Texas or, at the time of entry into the service, declared Texas as their home of record or was a Texas resident, select this checkbox.

Review or select the year and term to which the exemption applies.

Review or enter the number of Veteran's Affairs hours used during the year and term.

Review or enter the number of Hazlewood Exemption hours used during the year and term.

Review or select a classification for the student's Hazlewood Exemption hours.

	If the hours type you need is not available, select Maintain Definitions from the right-click window to open the Maintain Definitions window where you can add or edit the drop-down list values.
	Keep in mind that the values in this drop-down list are to be reported to the State of Texas as part of the Hazlewood Exemption Database Report. If you change any values, you must map each to a valid value required for the report. You can use the Institutional Reporting Definitions window to define Institutional Reporting fields and map them to J1 values.

Click this button to open the Student Billing Exemptions window where you can review, update, or add any billing exemptions or waivers for the student.

The button is displayed only if you have access to the Student Billing Exemptions window.

How To

1. Access the Hazlewood Recipient Information window.

The student's ID number and name display automatically.

2. Select the student's relationship to the service member.

	If you choose "Self," the student/service member's biographical information displays automatically.
	If the relationship you need is not available, select Maintain Definitions from the right-click window to open the Maintain Definitions window where you can add or edit the drop-down list values.
	Keep in mind that the values in this drop-down list are to be reported to the State of Texas as part of the Hazlewood Exemption Database Report. If you change any values, you must map each to a valid value required for the report. You can use the Institutional Reporting Definitions window to define Institutional Reporting fields and map them to J1 values.

3. If necessary, enter the service member's first name, middle initial, last name, Social Security number, date of birth, and zip code.

If the student's Relationship to Service Member is "Self," these columns are automatically populated. If the relationship is anything other than "Self," enter the service member's information.

4. If the service member is a Texas resident, select the Resident of Texas? checkbox.

5. If the service member is in default on any student loans, select the checkbox labeled Is Service Member in default on any student loan(s)?

6. If the service member entered the service in Texas or, at the time of entry into the service, declared Texas as their home of record or was a Texas resident, select the checkbox labeled Does Service Member meet initial time of entry requirement?

7. In the Hazlewood Exemption Term Information group box, review or update the student's exemption hours by year and term.

To create additional year and term rows, select Add Row from the right-click or Options menu.

a. Review or select the year and term to which the exemption applies.

b. Review or enter the number of Veteran's Affairs hours used during the year and term.

c. Review or enter the number of Hazlewood Exemption hours used during the year and term.

d. Review or select a classification for the student's Hazlewood Exemption hours.

	If the hours type you need is not available, select Maintain Definitions from the right-click window to open the Maintain Definitions window where you can add or edit the drop-down list values.
